You should request certified copy of your marriage license. You may request a certified copy online. You may also request a copy in person at the Marriage Bureau 8:30 a.m.-4:00 p.m. Monday through Friday. The Marriage Bureau is located in the Moultrie Courthouse, 500 Indiana Ave NW, Room JM 690. Submit the certified copy to the Social Security Administration along with other required documents. Once you’ve changed your name with Social Security Administration, you can change your name at the Department of Motor Vehicles (DMV).
